Course Enrollment Forms

 

Course Enrollment Forms allow us to easily enroll you in our Courses.

 

The following are examples of the Forms you would submit and sign when enrolling in our Courses. Please note we use Excel Spreadsheets and DocuSign links to enroll School Districts and individuals in our Courses.

Master License Agreement

 

Use the link below to view a Preview Copy of our current Master License Agreement. This agreement is embedded in our District and Participant Forms, and must be signed before enrolling in an Ashlock Consulting Course.

Group Enrollment Sheet

 

Use the link below to view a Preview Copy of our current Group Enrollment Sheet. All Districts and individuals enrolling in an Ashlock Consulting Course are required to download this spreadsheet, fill out the information, and send it back to us via email and through DocuSign.

 

To correctly fill out the Group Enrollment Sheet you will need to enter:

  • The date

  • Your School District/Region

  • Your School Site (if registering for a Tutorial)

  • The name of the Course you want to enroll in

  • The First + Last Name of every participant you want to enroll

  • The email for every participant you want to enroll

Renewal Forms

Ashlock Consulting provides Renewal options for some of our Online Courses. Below are the forms and DocuSign links necessary for completing a Renewal for school agency or individual.

 

Renewal Process

 

Note: You must complete separate renewal forms for each individual course. If you are submitting enrollment for a Bundle, such as Arizona DTD Bundle, you can select that as an option. Renewals are typically for 365 days after paperwork is signed, though this is subject to our Master License Agreement and previously agreed contracts.

 

Follow the below steps to submit your Renewal Form:

1. Determine the number of enrollees or School Sites you want to renew.

2. Download the ACI Renewal Sheet, and enter the individual name(s) and emails or School Sites you want to renew.

3. Get a finished Purchase Order for the number of Renewals, or confirm you will pay via credit card when you submit the Docusign.

4. Click this DocuSign Link, which will allow you to enter all your renewal information. 

5. Wait 5-7 business days for our staff to activate your renewals.

Certificate Forms

Ashlock Consulting provides Certificates for completing our courses. Below are our Certificates. 

 

Oregon Dyslexia-Related Training Certificate

 

Before you receive an Oregon Dyslexia-Related Training Certificate, you must:

1. Complete the REC A Course and receive a Record of Completion.

2. Receive a score of 80% or higher in the REC A Final and receive a Record of Completion.

If you have not done the above, please review our Reading Expert Certification—Series A Course.

 

Follow the below steps to receive your Oregon Dyslexia-Related Certificate:

1. Download your REC A Course and REC A Final Records of Completion.

2. Open this Certificate Request Form. This link takes you directly to DocuSign, which will ask for your name, email, School District, and signature. This is required.

3. After clicking the link above, enter the your name and work email address in the DocuSign page.

4. Fill out the DocuSign form, including your name, work email, school district, etc.

5. Upload both REC A Records of Completion to this DocuSign Form and submit. (This is what you downloaded in Step 1.)

6. Please wait 5-7 business days for us to email you your Oregon Dyslexia-Related Certificate.

 

Arizona Dyslexia-Related (DTD) Certificate

 

Before you receive an Arizona Dyslexia-Related Training (DTD) Certificate, you must:

1. Complete the REC A Course and receive a Record of Completion.

2. Receive a score of 80% or higher in the REC A Final and receive a Record of Completion.

3. Complete the REC B Course and receive a Record of Completion.

4. Pass the REC B Final and receive Record of Completion.

 

If you have not done the above, please review our Reading Expert Certification—Series A Course and our Reading Expert Certification—Series B Course.

 

Follow the below steps to receive your Arizona Dyslexia-Related (DTD) Certificate:

1. Download your REC A Course, REC A Final, REC B Course, and REC B Final Records of Completion.

2. Open this Certificate Request Form. This link takes you directly to DocuSign, which will ask for your name, email, School District, and signature. This is required.

3. After clicking the link above, enter your name and work email address in the DocuSign page.

4. Fill out the DocuSign form, including your name, work email, school district, etc.

5. Upload all four Records of Completion to this DocuSign Form and submit. (This is what you downloaded in Step 1.)

6. Please wait 5-7 business days for us to email you your Arizona Dyslexia-Related Certificate.

Arizona K-5 Literacy Endorsement Certificate

 

Before you receive an Arizona K-5 Literacy Endorsement Certificate, you must:

1. Complete the REC A Course and receive a Record of Completion.

2. Receive a score of 80% or higher in the REC A Final and receive a Record of Completion.

3. Complete the REC B Course and receive a Record of Completion.

4. Pass the REC B Final and receive Record of Completion.

5. Complete the Research Anthology Checkpoint Assignments Course and receive a Record of Completion.

6. Complete the Analyzing Student Data Course and receive a Record of Completion.

7. Complete the Student-Centered Master Schedule Tutorial and receive a Record of Completion.

 

If you have not done the above, please review our requirements for the Arizona K-5 Literacy Endorsement.

 

Follow the below steps to receive your Arizona K-5 Literacy Endorsement Certificate:

1. Download Records of Completion listed above.

2. Open this Certificate Request Form. This link takes you directly to DocuSign, which will ask for your name, email, School District, and signature. This is required.

3. After clicking the link above, enter the your name and work email address in the DocuSign page.

4. Fill out the DocuSign form, including your name, work email, school district, etc.

5. Upload all seven Records of Completion to this DocuSign Form and submit. (This is what you downloaded in Step 1.)

6. Please wait 5-7 business days for us to email you your Arizona K-5 Literacy Endorsement Certificate.

 

Transfer of Reading Expert Certification

 

To request a transfer of Reading Expert Certification from one school district to another you must follow the three steps below.
